Step-by-step plan for JGZ employee

Step-by-step plan for implementation of GigaFaaf-to-go!

When a JGZ employee works with GigaGaaf! wants to get started and is looking for collaboration with oral care practices.

        1. View the “KNMT practice map Dentist and youth doctor will help you on your way”.
Collaborate with youth health care? The practice map will help you get started | KNMT
        1. Map oral care practices in your area.
This can be done via website: Dentists via: https://tandartsregister.nl/
Dental hygienist via: Find a dental hygienist - NVM Dental Hygienists - Consumers
Oral care practices that have informed the Ivoren Kruis that they work with the caries prevention method Gewoon Gaaf can be found at Find a Gewoon Gaaf practice
        1. Please contact all dental care practices to ask whether they are willing to collaborate. It is important that all oral care practices in your region have the opportunity to participate. In this way, competition for patients is prevented.
See the sample letter - Download sample letter requesting cooperation with dental care practices
        1. If you intend to collaborate, it is important that you are aware of each other's wishes and preconditions with regard to that collaboration.
Map out the preconditions and wishes of both parties (oral care providers and JGZ) for cooperation.
          – See the Guidelines for the questionnaire for cooperation in dental care practices – JGZ.
          – See for this: Blueprint questionnaire to JGZ CB GigaGaaf to Go.
          – See for this: Blueprint questionnaire for GigaGaaf to Go oral care practice.
        1. Discuss the findings with the contact persons of the oral care practices.
        1. Adjustments to digital child file
Questions can be included in the child file regarding: brushing teeth (BDS 188) and visiting the dentist (BDS 190).
Names and addresses of dental care practices that have indicated that they wish to receive young children can be read into the system.
Providing oral care can be created in the file as a task, activity or referral. It is important to include a follow-up (at the next child consultation or at the 11-month consultation) to inquire whether the child has actually been to an oral care provider.
The letter to the dental care provider is drawn up according to the standard procedure in the data system, as is also customary for other referral letters. The letter must of course be sent in a secure manner (for example via healthcare email).
          Sample letters, see:
          a. Sample letter A Referral to parent to his or her own dental care provider.
          b. Sample letter B1 Referral to dental care provider, call child after 4 weeks.
          c. Sample letter B2 Referral to new dental care provider.
          d. Sample letter C about referral by dental care provider to parent.
          1. Provide the JGZ with sample letters for referrals. Depending on the form of collaboration, different concepts can be chosen:
            – when using Zorgmail or other secure email system: see sample letters above.
            -When using hard copy references
            a. Example referral card Parent to make an appointment with an oral care provider (pdf)
        1. Once the collaboration has started, it is a good idea to send a letter indicating how many dental care practices are participating and what the initial experiences are and announcing an evaluation after approximately 6 months.
Sample letter D to dental care provider after start of collaboration.
